INSURANCE AND BILLING

• Copay: please be prepared to pay this at the time of your appointment
• Deductible: we will send you a bill in the mail for this
• Referral Forms/Prior Authorization: some insurance require you to have a referral form from your primary care physician before you can see one of our specialists. Also please note, due to some insurances requiring prior authorization before approving testing you may need a few days before we can schedule a test. Also, insurances do not allow performing more than one test a visit.

TEST INSTRUCTIONS AND INFORMATION

We request that you arrive at least 15 minutes prior to a scheduled test.

If you miss a scheduled test without providing 24 hour notice to our office you may be charged a fee.

If you are running late, please call us so that we can make the appropriate arrangements. Please note that your test may be rescheduled if we are unable to accommodate you.

For Echocardiograms/Holter/Event Monitors:

• Do not use any lotion or powder on your skin on the day of your visit
• Wear to a 2 piece outfit (women)
• If you would like to have a chaperone during the test please do not hesitate to request one. Friends and family are not allowed in the testing areas

For Exercise Stress Echocardiograms/Exercise Treadmill Tests:

• Wear comfortable work-out clothes/shorts/pants and running shoes
• Do not use lotion or powder on your skin on the day of the test
• Take all of your medications unless otherwise instructed
• Refrain from eating 2 hours prior to your test; you may drink a sip of water in case you need to take a medication
• You may request a chaperone during your test

For Exercise and Chemical (Lexiscan/Adenosine) Nuclear Stress Tests:

• Wear comfortable, work-out clothes/shorts/pants and running shoes
• Do not use any lotion or powder on your skin the day of your test
• Take all your medications unless specifically instructed
• 24 hours prior to the procedure please refrain from eating or drinking any products that contain caffeine or nicotine in them including coffee/tea (including decaf), chocolate, tobacco related products/e-cigarettes, and any drugs that contain these substances. If you take theophylline on a regular basis please hold your dose at least 12 hours prior to the test as this may interfere with the test results.
• Please note, there are times when exercise nuclear stress tests must be changed to chemical nuclear stress tests with short notice. Therefore, it is advised to not use caffeinated products or nicotine for all nuclear stress tests.
• Refrain from eating 2 hours prior to the test
• You may bring a small snack to eat with you so that you can eat after the test is complete

PRESCRIPTION REFILLS

We are happy to fill your cardiac medications prescribed by our physicians. In order to ensure that you receive your prescription in a timely manner we require that you call us and leave a message at least 5 business days in advance of your next refill. Please be advised that any medications refills after business hours Monday through Friday 9:00am to 5:00pm will not be filled by the on-call physician.

If a critical medication needs to be refilled AFTER office hours or on the weekend, you should contact your pharmacy and ask your pharmacist to contact the doctor on call directly (at 301-670-3000) to request the medication refill on your behalf.
